Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Homework assignment to client/group
SP-C005 Home Practice after Intake. 04/28/2024
1. Tapping on bones around eye sockets. To release tension around the eyes.
o Softly tap around the surfaces of the orbital bones, both clockwise and counter-clockwise.
Nice long exhales. Could sigh “Ah” on exhale, or breathe out of the nose and extend the length of exhale without straining.
2. Full Belly Breathing. To down regulate the nervous system, and revitalize with a balanced quality of calm.
o Sense and observe. Begin by bringing your attention to the physical sensation of the breath in the body. Notice the qualities of the breath: shallow, deep, choppy, smooth, easeful, tense, quick, slow, expansive, constricted, and/or anything else you notice. Sense the body moving with the breath.
o Shape the breath. For about five breaths, invite the breath to be steady, smooth, and easeful. Consciously release tension in the breath.
o Transition to Full Belly Breathing. Inhale and let the belly expand, letting the breath flow smoothly from the belly, through the chest, throat, mouth, and nostrils. Exhale and let the breath flow outward with a slight compression of the lower abdomen at the bottom of the exhale. Continue for as long as you’d like.
If thoughts filter in, return to the sensation of the breath in the body.
3. Fingertips to the divot of the throat, hand on the sternum. Calming. Can combine with #2 above.
4. Crocodile with Humming Breath. Vagal nerve regulation. To release mental tension and calm the mind.
o Head on arms invite thoughts to flow out of mind, down into ground. Transition to humming breath. Make humming sound on exhale. Inhale naturally.
o Can have support under ankles or not.
5. Legs Up the Wall.
o Legs up the wall or on a chair seat. Aids circulation, calming.
Activities
Center, ground. Discusses intake form responses. Physical and breath assessments. Based on observations and client intake discussion: tapping around the eye sockets while exhaling with a sigh, bhramari pranayama in prone crocodile, extended exhale with hand at heart center, supine windshield wiper legs, legs up the wall.
Client/Group progress summary
N/A Intake, no prior session.
Reflection and self-evaluation
Client noticeably fatigued and overwhelmed with caregiver role, caring for her parents (mother recently diagnosed with breast cancer for 3rd time in her life) and her infant and preschool grandsons. Suggested practices the client can integrate into her daily PT exercise routine.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session
Focus on the breath and linking breath to movement in her PT exercises. Include moving awareness through the body/body scan and full guided relaxation in each session for this client. She needs the rest.
