Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

Homework assignment to client/group

He did not practice 3 homework assignments because he was traveling, I only added alternate nostril breathing. I will follow up on these at the next session.
As for #2 below, he said he liked this one, “I trust my body’s ability to heal and restore itself.”

1. I sent him a sample gratitude journal template and encouraged him to begin his gratitude journal.

2. Japa meditation, I provided him with a positive affirmation as an example and suggested he practice it daily.

3. I gave him step-by-step instructions for mindful walking because he enjoys walking.

4. Alternate nostril breathing, 2-3 mins 2/day.

Activities

Today was 10 days since our previous session because Dan had spent a week traveling to visit his daughter and family. I began today’s session by asking him how he was feeling and if he had noticed any physical or mental changes or challenges.
Since he said he relaxed and enjoyed it last time, I guided him through breath awareness. Following this, I led Dan through a body scan, which left him feeling deeply relaxed and more aware of his body.
After the body scan, I guided him through joint movements on the chair. We then focused on slow shoulder and back movements, similar to Tai Chi, as he mentioned pain in his right upper back. He recognized the movements and commented, "This is like Tai Chi." He said he practiced Tai Chi 1 semester before his cancer surgery but
he won’t be able to do it due to his balance problem.
Next, I asked him to stand up for the balance assessment. Due to his neuropathy, he was unable to stand with his right heel above his left ankle without holding onto the chair for support.
We continued with a few minutes of eye movements followed by a 15-minute session of Yoga Nidra. I explained how to practice alternate nostril breathing and the session concluded with alternate nostril breathing with affirmation. Overall, he felt more comfortable and engaged with the practices.

Client/Group progress summary

Dan did not have time for practice because he was traveling for the past 10 days. However, he has understood the benefits of deep, slow breathing and learned how to use it effectively. While driving in Calgary, he found the experience stressful but managed it well by pulling over, closing his eyes, and breathing deeply.
He said, "Driving in a big city is stressful. I pulled over, closed my eyes, and breathed deeply, which helped redirect my thoughts and allowed me to refocus."
I asked his impression of Yoga Nidra, he said “That was most dramatic, when I first lay down my back hurt right on incision but the more we go through each part of
the body, then it went away. It's amazing, that's faster than any drug I've ever taken so far.
He is learning how to relax and manage his pain quite well.

Reflection and self-evaluation

I was glad to hear that Dan was able to use the breathing technique to calm his mind while driving. Additionally, he is experiencing the benefits of Yoga Nidra.
I believe this care plan is helping Dan not only relax his body and mind but also enhance his physical, mental, and emotional well-being by incorporating new daily routine practices.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I would like to add balancing practice to our next session because he mentioned that he would like to improve his balance and I noticed that Dan misses his Tai Chi practice.
I plan to add this practice after JAPA (joint movements) practice.

I will also ask for his feedback on the gratitude journal to determine if this practice is benefiting him or if it requires further attention or adjustments.
