Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

Most of the adaptations made were based on time limitation such as skipping a balancing pose to be able to include pranayama and meditation and then on the following session spending time on the balancing pose. Otherwise, client was able to successfully practice all tools provided in the session.

Client/Group Goals

Client feels ease in her mobility and is currently pain free.

Goal revisions discussed to include - safe transition from standing to sitting to standing and continue adaptive yoga to where client will feel comfortable attending a gentle yoga studio class.

Physical level

Client's mobility has become more easeful; low back, shoulder and hip pain are gone; has not experienced the emotional pain in sacral area;

Energetic level

energetic level remains the same due to heat and client having the flu; however, when client practices nadi suddhi client feels energized.

Emotional

client expressed biggest takeaway from the four locks and keys was that not everything requires a response, this awareness has brought client much peace to her relationship with her children. She has not experienced the emotional pain when disciplining one of her children since session#3.

Intellectual / Sense of self

Client feels more connected to the physical body and the self. Client is putting together a support system that works for her, she is taking the steps in prioritizing self-care needs; Client is teaching her children what she has been learning in the sessions and it's been beneficial to them as a family, she feels this is deepening their connection.

Spiritual orientation and support plan

Client is very connected to her well-being and to the well-being of family. Client feels support from her family and has a deep spiritual connection and acceptance of challenges presented.

Additional Information
Feedback received from client/group, anecdotal or written

Session #3 - client expressed how happy they were every time we finished because she felt so good and energized.
Session #4 - loves the breathing exercises because she can do them anywhere and she feels energized, very appreciative of the modifications to the poses; these sessions are important to her to reset; felt a release on the shoulder after doing the triangular forearm support addressing her shoulder pain.
Session #5 - took care of elbow pain by doing the joint freeing movements; has been practicing nadi suddhi to fall asleep.

Sample of homework given between sessions (after initial homework)

Intake: Daily, 5-minute standing/grounding/centering/movement sequence and 5-minute guided meditation to address client's request.
Session 2 - incorporate joint freeing movements practiced in session to daily meditation practice - either before or after to benefit mobility.
Session 3 - nadi suddhi 5min, twice a day until following session for preparation to meditation; locks and keys practice as applicable throughout the day to improve relationships; incorporate sacral area movements to established asana practice to address the sacral chakra.
Session 4 - nadi suddhi w/ pratipaksha bhavana 5min, twice a day until following session; continue w/asana and meditation; triangular forearm support 30 seconds, 5 times within 10 days.
Session 5 - continue with established practice; think about revising goals such as safely transitioning from standing to sitting to standing; tree pose 10 seconds on each leg daily to increase balance;

What you would change with benefit of hindsight

Slow down; I will prioritize the practices that are most essential to address client's needs based on check in concerns. I will schedule longer sessions, if possible. Send session summary e-mail within 24 hours - preferably within two hours after session.
