Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

-Practice shoulder rolls during your day - standing at kitchen sink, standing in checkout line, etc.
-Abhyanga at bedtime
-Connecting with Koshas - select a specific Kosha that’s calling for your attention - then focus on that one for a day or a couple of hours during your day.

-Asana Warmups- 6-posture sequence w/ breath awareness
-Spinal Alignment / Awareness - from Mountain Pose - with eyes closed or soft gaze - tune in as you ground into the earth - feeling contact with all four corners of your feet - mentally scan from feet to ankles, knees, pelvic area, abdomen engaged, chest, shoulders, neck and crown of head. Cultivating proprioception and earth connection.
Practice Shoulder Rolls - 2 sets of 10 in both directions
-Pranayama - Finding a relaxed and alert seated position w/ hands on belly. Deep inhale for count of 3 to 5, exhale Ujayi breath for count of 6 or longer – continue for 3 to 5 minutes. This is intended to help reduce anxiety and connect with your belly, intuition.
-Mindfulness - Kosha review and discussion re our multi-dimensional nature
-Lifestyle Practice - Abhyanga for body appreciation and connection. Bedtime ritual

Client/Group progress summary

Client is embracing all that we explore. She's loving the Asanas for her upper back. Also having fun with the Koshas and how they work together. She understands the importance and benefit of Pranayama not yet incorporating that into her daily routine.

Reflection and self-evaluation

All in all, I think client is gaining a lot of benefit from our sessions. She's so eager to try new things. I'm wondering if I'm skimming along surface to keep things interesting by introducing new tools and practices. Wondering how I might go deeper or if it's a mix of core practices and new ones to explore.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Review / refine your Asana sequence and introduce 1 or 2 more asanas that will augment / complement objectives re reversing Kyphosis and strengthening upper back.
Pranayama - check-in re breath practice and possibly introduce one more.
