Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client/Group progress summary

The client just came back from her one-week vacation. She did play golf on most days. She have been paying attention on how she felt (we discussed about this during the assessment session). Overall, the pain is not bad. She notices that it is there, but not too the level that is unbearable. Average PEG scores for questions 1 and 2 4.5 out of 10. However, the score for question that how her pain interfered with her general activity is 7 out of 10. The client reported that she cannot work in the garden as long as she used to. Global health score ranges from good to very good. She feels anxious sometimes.

Reflection and self-evaluation

The client can carry out most of her daily activities, although she cannot spend hours working in the garden like she used to do. We discussed not to push through pain. I remined her repeatedly to listen and be kind to her body. She does home practices that I gave her during the assessment session regularly, although she finds that pranayama is challenging (she gets bored after awhile). I will continue to discuss with her about the benefits of pranayama.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn.
