Homework assignment to client/group

He has plenty of homework to do now, I added glute stretch into balancing
practice. Since the whether was not good past week where he lived, he said
he would resume his mindful walking. Overall, Dan showed dedication to his practice and a willingness to incorporate new techniques to improve his physical and mental well-being.

1. Balancing Exercise: I provided a sheet with images and instructions for balance exercises, reminded him to practice safely with a clear surrounding, and use a table or wall for support.

2. Eye movement: 1 or 2 times per day and as needed.

3. Gratitude Journal: I encouraged Dan to start a gratitude journal and advised him to add other items as well.

4. **JAPA Meditation with Alternate Nostril Breathing**: Suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

5. Mindful Walking: I gave him step-by-step instructions for mindful walking since he enjoys walking.

Activities

I began today's session by checking in with Dan about how he was feeling and whether he had noticed any physical or mental changes or challenges since our last meeting. He mentioned that there hadn't been any major changes, but he did share that he had started practicing the "chin-down-plus-larynx-tightening maneuver" and found it helpful. Understanding that achieving natural swallowing takes time, he expressed his commitment to continuing this practice.

To start our session, I led Dan through a centering exercise using three-part breathing, which helped him relax and focus on our practice. Building on his positive feedback from the previous session, I guided him through eye movements that he enjoyed and had been practicing daily since our last meeting. He expressed that he found the practice harmonious and beneficial.

Moving into physical exercises, I led Dan through joint movements and slow shoulder and back movements, emphasizing the importance of deep breathing with each motion. We then focused on balancing exercises for approximately 10+ minutes, which Dan also committed to practicing at home. During the balancing practice, Dan noted feeling weakness in his glute muscles while performing hip flexion while holding onto a chair and standing. I provided him with exercises to strengthen those muscles and emphasized the importance of practicing them safely due to his balancing issue.

To conclude the session, I led Dan through a 15-minute session of Yoga Nidra and alternate nostril breathing. Observing his body release tension and witnessing him completely relax, as always.

Client/Group progress summary

Dan continues to demonstrate his progress every time I see him. He learned eye movement practice now. And he added this to his daily routine already, I believe this new practice will bring him another benefit to his well-being. For eye movements, he said visualizing the big clock in front of him was very helpful.
I understand it takes time for him to improve his balance, because of several factors,
his neuropathy from chemotherapy, lack of exercise after cancer surgery, and his body type. But I expect his improvement of balance over time.

Reflection and self-evaluation

I'm glad to hear his progress report every time I see him. This was his 5th session, including the intake. I can feel his commitment to the practice. A good example is his ability to observe his feelings and the effects of the practice, expressing his felling and observation in his own words. Instead of simply saying "I like it" or "It was good," he described his experience as "harmonious” or “dramatic”. I can tell he is diligently focusing on every home practice assignment.
Overall, the program is working well, and I expect to see even more improvement. I plan to continue seeing him for at least a few more sessions.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions. For the physical practice, I will add a few exercises specifically designed to strengthen his lower body muscles, as he mentioned experiencing weakness in his glute muscles. In terms of wisdom teaching, I will encourage him to document any feelings, pain, or thoughts in a gratitude journal. This practice will help him observe and understand the connection between his physical body and his mind. By consistently recording his experiences, he can gain deeper insights into his overall well-being and progress.
