Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

Since her most recent medical appointment and the news of the surgery, I’ve introduced upper body strength building movements. Asanas focused on the hips and legs have changed due to limited range of motion and increasing pain. We continue discussing the Sutras, Yamas & Niyamas, and practice Yoga Nidra.

Client/Group Goals

The client reported that she feels relaxed after our sessions and temporarily experiences reduced pain. She is finding it difficult to generalize the breathing techniques and visualizations. She is trying to practice medication and pranayama daily, and is managing three days a week.
The first goal is modified since the degenerative condition has reached the point of pain at a level 7 on a scale of 1-10, 10 being the worst pain (client reported on 24.06.12). We continue to implement Goal two, to practice daily meditation and pranayama, as written.

Physical level

The client’s pain continues to worsen due to the degenerative nature of the condition. There appears to an increase in adverse outcomes, including depression, isolation, and sleep disturbances.

Energetic level

When we practice breath awareness the client reports feeling revitalized. When asked about her energy level, she states feeling a removal of stagnant energy, and a calmness.

Emotional

Client appears sad and uncomfortable due to the ongoing degeneraion of her hips. She reported today that she’s been on this ‘journey of discovery’ for 2 ½ years. Though the outcome is difficult to accept, she voiced resolve after getting an answer.

Intellectual / Sense of self

It’s a pleasure speaking to a client who has a strong sense of self, and an interest in yogic philosophy. The client follows up with suggestions on websites, papers, and videos that may be of interest whereafter we discuss the content.

Spiritual orientation and support plan

PAM appears to enjoy sharing her perspective on the Sutras I’ve introduced through her care plan. We have 4 more sessions planned and will discuss how I will continue to support the client.

Additional Information
Feedback received from client/group, anecdotal or written

PAM reported that the scripted Yoga Nidras were well-received. She was able to relax, she said. She told me her pain level was increasing; and, she was upset about not being able to get an appointment for surgery until August 2024. She shared that some of the asanas we’ve practiced were also recommended by her physical therapist and newer doctor.

What you would change with benefit of hindsight

I asked many times for help with a plan of action, an approach to the process, how many hours would be needed, etc. I’m a planner. I felt I was juggling my personal life which had exploded while trying to create a structure for my professional life as well. It was a lot to deal. I set aside the practicum work while I strategized my daughter’s new illness, and then my firing. I didn’t have the wherewithal to create a structured approach to my practicum. With it lacking, I felt paralyzed and was unable to move forward. I believe I did what I needed to do. I just wasn’t ready.
