Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Discussed client's court session from last month regarding child support and custody. Discussed her trips out of town for a poetry festival (her 50th birthday trip) and for graduation from her expressive arts certification program. Discussed aspects of her work she dislikes and how to handle those.

Client/Group progress summary

Client had been doing very well with her clean diet, but has become discouraged because recent blood work revealed that her cholesterol and blood sugar levels have increased. She was so upset about this that she said "screw it" and started eating whatever she wants. We talked about how these types of set-backs are totally normal - the key is simply to see them, and decide to get right back on track. She states she is ready to do this - which is very different from how she was in the past when she would completely give up.
Client is beginning to accept "how she is" as a deeply spiritual person (almost to the point of being a nun), and that she doesn't have to apologize to anyone about who she is and how she lives her life.

Reflection and self-evaluation

I'm still really tired but I always enjoy working with this client. She is so much like me that I find it easy to meet her where she is. She feels comfortable with me because she knows I understand and I will not judge or try to change who she is. I enjoy seeing her do well and make progress even when it's very slow.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
