| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |

| Adjustments and adaptations you made to your care plan, | The biggest adjustment was ending our sessions. Initially I had planned eight sessions with her, but with the decline of her mother-in-law's health, we were only able to complete four. |
| Client/Group Goals | Client reported feeling more steady and strong, with greater awareness of core engagement in standing asana. Client also reported no back pain during standing asana practice. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | Client reported reduction in back pain, and overall comfort in moving every day - whether taking her morning walk, her arm and core-focused exercises, or standing asana practice. |
| Energetic level | Client reported managing her energy well. Client enjoyed practice of nadi suddhi as a calming and energizing practice. |
| Emotional | The client felt she was better managing her emotions around her cancer and her mother-in-law's declining health. She was able to notice sensations present in her body during awareness practices, asana practices, and during her morning walks. She was able to allow herself to feel emotions without judgement and with offering self-compassion. |
| Intellectual / Sense of self | The client's meditation practice was established - steady and consistent. As she explored various guided meditations, moving meditation as well as meditation on the breath, she seemed to find moments to rest in the awareness of her vijnanamaya kosha. |
| Spiritual orientation and support plan | Client considered herself Christian, and appreciated when I would connect teachings from the sutras to passages of scripture. |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | Overall client was experiencing less pain in her low back, more stability and confidence in her body, and felt good moving every day. The client enjoyed the various pranayama practices, and felt that her energy was more steady throughout the day. |
| Sample of homework given between sessions (after initial homework) | Continue practicing Nadi Shodhana both at times where she feels the need to increase her energy or mental focus, as well as when she's feeling anxious or agitated. Note what she observes both before and after the practice in terms of her thoughts, emotions and sensations in her body. Continue exploring the wall-supported Surya Namaskar as a great way to warm up and move her body. |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | 30 |
| What you would change with benefit of hindsight | I did not get a chance to craft the guided imagery for the client that I had intended to. That was a practice I wanted to include more in my practicum. Hopefully I will be able to offer it with my future clients. |
| Did you enjoy your service? | Yes. It was an honor to work with Anne. I admired her calm strength, her tenacity, and her desire to approach health from a perspective of preventing what she could prevent, maintaining what she could maintain, and pursuing tools to heal what she needed to heal. |
