| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |

| Adjustments and adaptations you made to your care plan, | While initially I thought the main goal would be on supporting her healing as she continued with her immunotherapy treatments, in discovering her passion to share what she has learned and support the recovery of others, I added more wisdom teachings from the sutras that aligned with what she has learned to apply through the 12 Steps of AA. |
| Client/Group Goals | Client felt she had learned more about yoga's benefits for her mind as well as her body - she enjoyed learning about the panchakosha model. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | Client reported feeling stronger, with reduced tension in her low back and shoulders/neck. She felt she learned ways she could modify asanas to reduce discomfort in her knees. |
| Energetic level | Client reported overall improvement in energy, and greater attention to how she needed to practice bramacharya each day in managing her energy well. |
| Emotional | Client found the wisdom teachings ( ex Pratipaksha Bhavana/sutras 2.33-2.34 and the Locks and Keys/sutra 1.33) helpful in regulating her emotions and calming her mind. She was also eager to share these with others in recovery. |
| Intellectual / Sense of self | Client is very sattvic in her demeanor and speech, modeling a balance of humility and confidence. |
| Spiritual orientation and support plan | Client has a deep spiritual foundation and loves to continue studying and exploring spiritual practices. |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | Client enjoyed learning about the Yoga Sutras and finding ways to incorporate them into her daily practices, as well as her teaching moments at The Healing Place. |
| Sample of homework given between sessions (after initial homework) | Created a document for the client linking the 12 steps of AA with each step of Surya Namaskar. (unable to attach to this note) |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | 20 minutes |
| What you would change with benefit of hindsight | I planned for 8 sessions because that was the YCAT outline, but I feel I could have offered the same support within 5 or 6 sessions. |
| Did you enjoy your service? | Very much so! |
