Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

Homework assignment to client/group

Continue with healthy diet and exercise routine.
Remember to drink plenty of water (she tends to forget this).
Take a break every 1-2 hours from work to perform her gentle asana routine.
She stated when exercising, she often feels unable to complete the last 1-2 repetitions and she's unsure if the block is physical or psychological. I counseled her to pause for 2 slow deep breaths at that point, then complete the reps.

Activities

Client has been packing, cleaning, and moving boxes, etc. for remodeling of kitchen. She has been busy/active for 8-9 hours a day which is much more than usual. She stated she also walked 30 mins 2x this week with friends and practiced the pelvic tilt exercise regularly. She practiced cow/cat and forward fold to relieve back pain. I had sent her a list of 35 fruits, veggies, whole grains, nuts, et cetera, I use as a foundation for a healthy diet. She stated she loved this and it helped her bring more variety into her diet this week.
Asana: chair and supine including strap stretches to relieve muscle soreness through her whole body including upper body.
Pranayama: extended exhales for stress relief
Complete yoga nidra with her legs propped on her sofa

Client/Group progress summary

Client did well this week with her selfcare routine. She seems more motivated to do what she needs to do for herself.

Reflection and self-evaluation

I had an easy time meeting the client where she was this week. Since she had a lot of tension and muscle soreness, it made sense to focus on relieving that. Toward the end of the practice, she stated she could feel so much more freedom of movement and the release of lower back tension/pain. She also loved the yoga nidra.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
