Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Seated class included:
Centering with breath and body awareness, guidance on diaphragmatic and 3 part breath that we revisited throughout.
Gentle warm ups - cat/cow, slide twist, cherry picking w/cross over body
1 slow seated sun sal
3 x seated sun sals with breath cues

Ankle movements, Chair boat with cross crawl
Chair Chair pose with options (feet press into floor, lift slightly off seat, etc)

Client/Group progress summary

As a group, good coordination and balance today. I notice a difference in those who attend more regularly.
Everyone appeared calm today, moving generally slow, not much sharing, some were absent.
They appreciated the Peace begins with me meditation today.

Individual connection, feedback, and observation reveals that one just found out about a new ailment her husband has, one shared that she feels so much better after class (chronic pain), one was on time for the first time today, one got on the floor for savasana and put her legs in the chair (huge deal as she has 2 botched hip replacements).

Reflection and self-evaluation

I’m almost at the final report for this and all the groups. I think I did a good job incorporating a little of everything we’ve learned in the workshops.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I’m not planning anything special for next time, as they don’t know it’s the last session I’ll report on. We will keep going. After next week, I may recheck their goals, catch up with any new people for assessment, and outline the next several workshops.
