| Homework assignment to client/group | I explained and assigned 6 poses to add to her routine to improve her IBS condition, they are: I also suggested creating an altar & daily prayer |
| Activities | I began today's session by checking in with Reina about how she was feeling and whether she had noticed any physical or mental changes or challenges since our intake session. She stated |
| Client/Group progress summary | This was the 1st session after assessment, I did not see any progress at this time. |
| Reflection and self-evaluation | She maintains a healthy lifestyle, she has 5/week yoga practice at home and at the studio, also she plays pickleball and aquarobics and the gym. I think that’s g great way to reduce her mental stress and shift her mind away from her concerns about her husband’s health. Another problem she was expressing would be improved by creating and maintaining an altar. It can be therapeutic, providing a sense of peace. It helps Reina express her emotions and memories, contributing to emotional healing. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will continue to follow this care plan for the next several sessions. I will check with her about her sleep and agitated dreams. And any benefits from homework assignments to determine the need for any modification to the care plan. |
