Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

The joint freeing series concentrated on spinal movements, shoulder, neck, head, wrists and hands. All elements of the care plan were included at some point over the five sessions together.

Client/Group Goals

• Any progress towards goals
Personalized guided relaxation helped the client to integrate a range of emotions around her past life experience in various phases of her life. As she put it, “There is sweetness in the sadness.” The client’s neck ROM remained the same. She now has a set of movements to relieve stiffness and possibly increase ROM over time.

• Review and revision of goals.
The general goals remained the same, the overriding theme came to be grief and loss, affecting all aspects of her being.

Physical level

Ease in neck and hands. More stable balance with awareness to create a stable, solid, grounded base in asana.

Energetic level

The client moderated her activities to maintain an energy reserve. Extended exhale and full diaphragmatic breathing practices brought calm and ease with aliveness.

Emotional

The client’s sense of loss and grief is eased by retelling stories of her loved ones. She acknowledges her resiliece, attributing some of her strength to yoga pracitce. She consciously lives life to the fullest each day, letting go of sorrow to the extent possible at any given moment.

Intellectual / Sense of self

Dancing Ganesha was quite challenging for the client at first. Over time, the brain/thinking challenge dissipated. As the body became familiar with the movements, she was able to flow fluidly through the movements, embodying fearlessness with ease. A lifelong learner, the client enjoyed sharing the newfound knowledge she gained over the course of our sessions.

Spiritual orientation and support plan

The client’s religious affiliations and her love of her plants gives her strength and a continuous sense of wonder and awe of both the natural world and spiritual realm.

Practice for hand mobility, neck stretches, dancing Ganesha hand movements for brain challenge, and integrating rest time into her day. Follow Ann Swanson videos for guidance with mobility movements.
