Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Extended exhale practice. Gentle stretching throughout the work day.

Activities

Check-in, arrive, centering & grounding. Seated warm-up, including neck. Hip focused stretching. Leg swings, internal & external rotation of leg at hip joint, IT band stretches, hamstring stretch with heel on chair seat + fold, quad stretches, seated pigeon, demonstrated standing back bend w/hands @ sacrum, joint freeing for elbows, wrists, fingers,, ankles, toes, feathering touch across sides of neck. Breath awareness. Extended exhale guided pranayama: inhale 4, pause @ top of inhale, exhale 8. Could increase count to 6 in, 12 out. Guided meditation with healing, medicinal water. Cheri Clampett Therapeutic Yoga manual pp 118-119.

Client/Group progress summary

The client remains calm and at ease while sustaining long hold stretches. Prior shoulder injury inhibits ROM.

Reflection and self-evaluation

Client was fatigued, did not sleep well the night before. Pain level low. Eager to stretch and meditate, prepare for their afternoon.

Bring in wisdom teachings, maybe through choice of meditation, mantra. Choose a couple of mantras for the client, see what resonates with them. Offer an accompanying image to go with the mantra. Client is a visual person.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
