Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client and I will keep in touch via email and texting as needed. She will be in my chair yoga class for the next two Tuesdays.

Activities

Discussion regarding the client's exercise routine. She stated she has been feeling very tight in hips/legs. She has been gardening alot lately and really enjoying that as a creative outlet.
Asana: supine hip/hamstring stretches and twists using a variety of props including a bolster, strap, and blocks.
Pranayama: breath awareness; slow, deep breathing
Yoga nidra: took her through body and breath awareness. When I said, "notice the interconnection of your physical body with the ground," she held her arm slightly off the floor and said, "floating."
Session 11 and 12
Leg strengthening work: chair pose, isometric high lunges, clam shells
Strap stretching, dynamic twists, and restorative Mountain Brook Pose (which she loved!) for posture; restorative modified child's pose, shoulder stand and twist
Session 13 and 14
Wall push-ups/plank, standing shoulder internal/external rotation, supine asana, passive backbend with rolled blanket; chest presses; slow deep breathing
Session 15 and 16
Strap stretches, restorative asana, yoga nidra, 3- part breath
Session 17 (final session)

Client/Group progress summary

Even though the client experiences anxiety/depression-like symptoms at times, she can very easily and quickly relax in yoga nidra. Today, in the quiet time, she said, "Thank you for this. I can see my spiritual practice hasn't gone anywhere." She was completely connected into God. She said, "This is not effortful." I still believe the client is in a classic dark night of the soul because she experiences strong resistance to formal spiritual practices of prayer/meditation, yet she effortlessly connects to God.
Session 12: Feeling much better physically, mentally, emotionally. Has been walking more frequently and easily - even as much as 1.5 hours at a time! Has started meeting rec center friends for lunch (something she has never done before) and enjoying it.
Session 15: Updated goals (upper body strength; balance work; emotional support; assistance with fitting artwork into her daily schedule). Client has made excellent progress on her previous goals.

Reflection and self-evaluation

I greatly enjoyed this time with the client today. I enjoyed seeing how the asana was relieving her tension/pain. I was amazed to see how quickly she could relax and connect to God. I can do this, too, but I know how rare this is for most people. Most people have no clue this is even possible. I told this to the client. Reinforced her understanding that being a contemplative person in love with God is a valid existence.
Session 17: Final session. I dearly love this client. I'm so glad she is doing much better.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
