Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

The client has arthritis of the right hip and osteoporosis of the femoral neck and focal osteoporosis of the ultra distal radius. The hip and groin pain have improved; She still notices the discomfort, but it is not as severe to the point that she cannot walk or function. The practice focuses on grounding and flow of movement and breathe

Client/Group progress summary

The groin and hip pain has reduced. However, she experienced pain after the last golf game; she felt slight discomfort during the game, but the pain came after she got home. We discussed overuse and repetitive movement. I suggested gentle yoga (pelvic tilt, leg up the wall/chair) and deep relaxation after the game; she usually stretches before the game. I also suggested to her that instead of playing 18 holes, she can skip some even when she does not feel pain while playing. We discussed how active meditation can help with relieving pain. She is motivated to practice meditation daily.

Reflection and self-evaluation

After meeting with the mentor, I planned the therapeutic session that addressed Vata/Pita imbalance by including standing asanas, forward bending poses and dynamic movement. I introduced Bhramari breath and explained the benefit of each breathing technique that she has learned so far: deep breathing, alternative nostril breathing, box breathing and Bhramari. I suggested to her to try them and see which one works best for her. I also explained the benefit of active meditation for pain management (training the brain).

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will conduct pain and overall well-being measurement to assess where the client is. Continuing physical practice that focuses on improving strength, flexibility and balance. I will end with a longer meditation (5 minutes).
