Approaching the client's plan of care from energetic body to alleviate physical

Perhaps reconsider NS to calm his anxiety and rather include the Humming (buzz) breath. It is an amazing practice; and a great benefit of the practice is that it can be done silently having profound effects. (Dr. Timothy McCall talks about this). For your client, using this technique, silently or not, rather than NS, can be a tool he could access anytime, anywhere, where as NS is a little more difficult to access in public places.

I like the approach of elbow replacement surgery, very good. Perhaps some more research on the benefit of the pranayama practices (especially since your key teaching included pranamaya kosha) would have been more compelling than the benefits of integrative modalities since we know how important it
is to see the client as a whole.
