It is clear from reading the entire report that you have understood the core concepts through the time with the client, your observations etc. However, the goal should be a SMART goal and you've listed mostly the plan in the area for the goal. You goal could be to use the extended exhalation to lower the level of anxiety and stress with a x min practice over x period of time and tell how the client will know if this has had an impact or could tell a difference - ie measurable. The amount of time and frequency can determine if it is attainable. No info about the client need be entered here.

Very individual approach (the combination of modalities with the client) and in your own observations and follow up - the idea of leaving eyes open is great. How about offering the use of something to cover the eyes? Another potential idea for Yoga Nidra would be to lift and drop the limbs without tensing first - the benefit of dropping is helpful to release the tension, or perhaps shaking to release instead?
