Did you advise her to practice pranayama techniques and to follow up with you if even a few minutes daily? (this may be a lot for both you and your client. (You might consider every few days or 2x's per week for a few minutes???)

This is why I was confused:

Other than from your manual and training I did not see any referenced research to your plan. I recognize that yoga therapy, yoga, and
endometriosis may not have a lot of research. This you can not in your TCA. You might research endometriosis and physical therapy. Google scholar or IAYT resources are very good. Here is an example of research I found.

Under "selected client and setting" rather than bullet points, keep to the specifics:
ie. Client is a F, age 47 with a medical history of . . . . . Client reports pain in lower extremity pain due to edema. . . .
Unless you feel this impacts on the client's wellbeing no need to include children or marital status. It is good to know medications that the client takes since that can give you some information as to side effects, but specific vitamins aren't needed especially since this is out of the scope of practice for a yoga therapist, unless you are a nutritionist and feel it is needed.
