| Short notes on time with client: | The activities seemed easy enough and the desired outcome is certainly an ideal and positive goal. | ||||||
| Follow up suggestions for your client (whether with you or on their own): | Keep working on the goals, they’ll be beneficial whenever they are worked ‘towards’. Maybe break them down into smaller goals, or try for any one or 2 each day. Or try incorporating one and when it’s firmly established, then add another. | ||||||

| Did you apply your intended plan once you met with the client(s)? Was the goal achieved? Explain. | The intended plan was applied. The goal was not achieved, although some progress was made. Those 4 simple changes were harder than they seemed like they would be. | ||||||
