Goal for implementation with client (Specific, measurable, attainable, relevant, time-bound relating to the client):

To reduce hypertension the client will do a five (5) minute OM chanting followed by a 20 minute yoga nidra session at least five times a week for two months.

Relevant Client(s) Details

The client is an active 61 year old male with hypertension on medication treatment. Client insists that the hypertension is inherited, and nothing will help except medication but is willing to try yoga. Walks daily for 30-45 minutes. No current yoga practice.

Practice/Activity (5 words or less) Amount of time (in minutes)

Seated, centering and grounding with breath Awareness 5 minutes
Warm up and sun salutations 10 minutes
OM chant practice. 5 minutes
yoga nidra 20 minutes
Sitali followed by meditation 10 minutes

What branch(es) of IY did you use? How does each support your goal/relate to the key teaching?

Japa – OM chanting – using sound to tune into the divine frequency and reducing the mind chatter to reduce stress and hypertension
Hatha – asana, pranayama, meditation, yoga nidra– to reduce stress, create body-mind connection and reduce hypertension
Raja – meditation – for gaining control over the mind and reducing stress and hypertension

Short notes on time with client:

The client is willing to try the practice but feels that the hypertension is family related and nothing can be done about it. The client enjoyed the practice and did feel very relaxed especially during the yoga nidra practice. After the session the client expressed feeling lightness in the mind and physical body.

Follow up suggestions for your client (whether with you or on their own):

The client is agreeable to practice daily as suggested and will be taking his blood before and after the practice once a week to see if there is improvement.

Reflection
Did you apply your intended plan once you met with the client(s)? Was the goal achieved? Explain.

Yes, the intended plan was applied, and the goal was achieved in the sense that the client understood and enjoyed the practice. The client was able to gain immediate benefit in this practice in feeling calm and relaxed.

Did you have to adapt anything in your plan? What lessons did you learn?

Yes, the session took a little longer than planned. I learned to have a visual timer available to me during sessions in order to avoid going over our time limit.

If you are faced with the same situation again in the future, would you approach it in the same way? Why or why not? What went well? What you might change and why? Summarize.

In the future I will be aware of the time it takes for each practice and adjust accordingly so that I do not go over the agreed upon time. The session went well, the client received immediate benefits from the practice. I need to work on my delivery of the session by providing detailed instructions in a more confident manner.
