Motion is lotion. Movement will lubricate the joints allowing the client to achieve greater ranges of motion more easily. Mobility work also strengthens the new and improved range of motion and therefore will have lasting effects. (Week 23, Muscular Skeletal Injuries)

Goal for implementation with client (Specific, measurable, attainable, relevant, time-bound relating to the client):

To improve body mobility, and therefore reduce pain in her lower back, the client will have two meetings of 30 min each and a 2X week practice by herself for one month, which includes breath awareness, 3 asanas and progressive relaxation. The methodology efficiency will be measured by her evaluation.

Relevant Client(s) Details

She is a 44 year old woman, currently unemployed actress, living in Ireland with husband and have 2 kids about (7 and 1). Last year she began to feel a pain in her hip/ buttock region that irradiates to her legs, which lead to depression and insomnia. She doesn’t have money to have an accurate diagnosis of her problem. She tried some sessions of physiotherapy but didn’t work well.

Report Information

How did you envision working with the client(s) to incorporate the selected teaching? (Define the plan)

The client will have two meetings of 30 min each and a 2X week practice by herself for one month, which includes:
1. Awareness of the breath, with dirgha swasan or 3 part breath, to activate the parasympathetic nervous system;
2. Pelvic tilt and trust (10x), to stretch the lower back and strengthen muscles of the abdomen, the deep muscles of the lower back and pelvic floor. "It develops the tone and freedom of psoas muscle, which can lead to separate movement of the five lumbar vertebrae." (Mukunda S.)
3. Rolling bridge, to strengthen spine, hips and thighs, and maintain good posture.(M.S.)
4. Virabhadrasana I with back heel lifted: "the toning of the hip extensors of the back leg may relieve discomfort of sciatica or lower back pain". (M.S.)
5. Progressive relaxation with visualization :The mind is able to rest, health is optimized, energy is restored. (Manual of Stress Management)

What branch(es) of IY did you use? How does each support your goal/relate to the key teaching?

Hatha yoga : the client was asked to practice: Asanas (annamayakosha), which stretch and strengthen the lower back muscles; and Breath awareness (pranamayakosha), which activates the parasympathetic nervous system, helping her to sleep better and to deal with pain and worry.
Jnana Yoga – During the sessions, the client is asked to be aware of the sensations in the body, mental perceptions (like colors, textures, etc) and feelings that may come up. This helped her in the process of Self-analysis, which will help her to release holding patterns and lead to the healing in the 3 subtler koshas.
Bhakti – During progressive relaxation, I used my client's Guru's words . Her devotion helped her (anandamaya kosha). Like Dr. Wayne says “For many, spirituality, faith, and religion are central parts of who they are. They can influence how you cope with trauma, fear, or loss"(How Healing Works)

Short notes on time with client:

In the first meeting, she was in a lot of pain and depressed. After discussing the intake, she was not able to practice the asanas except savasana (progressive relaxation). While exploring her hip, I asked her about sensations, colors, textures in this region. She reported that she visualized a cockroach. I asked what this image represented, and she said - "Fear". And then I told her to imagine instead a Disney movie and that now this cute insect was dancing with her in joy. And I said that it's ok to feel fear but we can change the way we see it (pratipaksa bhavana). She laughed and said that was feeling lighter and happier after the session.
In the second meeting, a week later, she said that the session gave her an incentive to go to pilates and she was not feeling pain anymore! We did the asanas this time. During the relaxation her baby came and she breastfed him.

Follow up suggestions for your client (whether with you or on their own):

Follow up suggestions, but she doesn't have to do all:
-Practice affirmations everyday, place them where you can see (bathroom mirror):
"God is within and around me, protecting me; so I will banish the fear that shuts out His guiding light." (P. Yogananda)
"I will go forth in perfect faith, in the power of Omnipresent Good, to bring me what I need, at the time I need it." (P.Y.)
-Practice exercises (yoga, pilates) at least twice a week.
-Observe negative thoughts and try to change to positive ones.
-Journaling before sleep every day. (could be just a word)
-Practice some minutes of yoganidra when you have insomnia (I sent some relaxing meditations to her)
-Include the kids in your sadhana, "play yoga" with them. (I sent some stories of Krishna that she can read to the kids).

Reflection
Did you apply your intended plan once you met with the client(s)? Was the goal achieved? Explain.

The goal was achieved, my client said she is feeling so much better. Her aunt call me to congratulate me and told me that she didn't see my client this way in many years. The successful result is due not just to the asanas, but to the whole plan. Beginning by the intake talk, just to have someone to really listen about one's problems can lower their pain. After creating a safe space, she could relax and surrender herself to what is, which creates a channel for healing in the body, mind and soul.
I applied the plan but had to change in order to adapt it to her case. I will explain next about the change.

Did you have to adapt anything in your plan? What lessons did you learn?

Yes, I had to adapt part of the plan. In the first meeting, I could see that the asanas were not working for my client, because she was in great pain. Then I focused more in yoganidra, deep breathing and deep relaxation with sensing the body, and affirmations at the end. This way she could create a safe space in her own body and mind to allow healing to happen.
I learned that I can use my tool box of yoga techniques accordingly to the situation of the client.
I learned also that the plan is just a trail and not a railroad. I don't have to stick to the plan but use it to guide me to know which tool would fit better in each situation. What I notice from the client reactions (expressions, movements, words) will be the guide during the yoga therapy.
Another lesson is to use my intuition or the buddhi mind to guide me during the process.

If you are faced with the same situation again in the future, would you approach it in the same way? Why or why not? What went well? What you might change and why? Summarize.

If I was faced with the same situation in the future, as a yoga therapist, I would do exactly what I did, because it worked very well.
My intention was to allow her to provide her own healing, through pranayama, self observation and asanas. These wholistic model is intended to restore the several layers of the body (koshas). Beginning with the body relaxation, the other koshas are affected. In integral yoga, we work from the gross to the subtle body, it's why it's good to begin sensing the body but also working with the second layer (pranamaya) through breathing techniques like 3 part breath. This way one kosha will affect the next and so on.
